Section 1: Company Information
Company Name*
Enter the full legal name of your company exactly as it appears on official tax and business registrations so it shows correctly on every paystub.
Select from Address Book
Select from Address Book to view and choose from your saved company profiles to prefill all company fields.
Company EIN
Enter your company’s Employer Identification Number (EIN) used for tax and reporting purposes; this helps ensure paystubs align with government records.
Company Logo
Upload your company logo to brand the paystub, making it easy for employees and third parties (like lenders) to recognize who issued the document.

Select your preferred paystub template. Click 'Change Paystub Template' to browse other template options.
Email Address*
Enter a valid company email address for communication.
Phone
Enter a company phone number to appear on paystubs for any inquiries and communication (optional).
Section 2: Employee/Contractor Information
Employee/Contractor Type
Select whether this paystub is for an employee (W-2) or contractor (1099) to ensure correct tax treatment.
Employee/Contractor Name*
Enter the worker's full legal name exactly as it appears on their Social Security card or tax documents.
Select from Address Book
Click to autofill your saved employee/contractor details from the address book.
TIN (Tax Identification Number)*
Enter the last 4 digits of the worker's SSN for employees, or SSN/EIN for contractors.
Employee/Contractor ID
Enter your internal identification number for this worker to help track and organize payroll records (optional).
Hire Date
Enter the date when this worker started employment or the contract began for accurate record-keeping. This ensures accurate year-to-date (YTD) calculations, especially for mid-year hires.
Calculate taxes based on a different state
Check this box to calculate state taxes for a different state than the employee's default work location. This is useful when employees are remote or work across state lines.
Use 2020 Form W-4 for federal withholding calculation
Check this box to use the current W-4 format (2020 and later) rather than the older allowances-based method.
Filing Status
Select the employee's tax filing status as indicated on their W-4: Single, Married Filing Jointly, Married Filing Separately, or Head of Household.
Step 2 - Multiple Jobs or Spouse Works
Enter the amount from Step 2 if the employee has multiple jobs or files jointly with a working spouse to adjust withholding.
Step 3 - Claim Dependents
Enter the total dollar amount from Step 3 for qualifying children and dependents claimed by the employee for tax credits.
Step 4(a) - Other Income
Enter non-wage income (interest, dividends, retirement) that should increase federal withholding from Step 4(a) of the W-4.
Step 4(b) - Deductions
Enter other deductions (mortgage interest, charitable contributions) from Step 4(b) that reduce taxable income.
Step 4(c) - Extra Withholding
Enter any additional amount the employee requests to withhold per pay period from Step 4(c) of their W-4.
Total Number of Allowances (old W-4 form)
Enter the total number of withholding allowances claimed by the employee from Box 5 of their pre-2020 W-4 form.
Exemptions
Exempt Federal
Select "Yes" if the employee is exempt from federal tax withholding.
Exempt FICA
Select "Yes" if the employee is exempt from FICA (Social Security) tax.
Exempt Medicare
Select "Yes" if the employee is exempt from Medicare tax.
State Information
Filing Status
Select the employee's state tax filing status from their state withholding form; this may differ from federal filing status.
Additional State Withholding
Enter any extra amount per pay period the employee requests to withhold for state taxes beyond the standard calculation.
Other State Tax Information
Enter additional state-specific tax details such as allowances, exemptions, state tax rates, or special withholdings required by your state.
Locale Information
Enter local tax information if the employee is subject to city, county, or municipal taxes based on their work or residence location. Include the complete address for more accurate tax calculations.
Section 3: Salary Information
Wage Type (Salary/Hourly)*
Select whether this employee/contractor is paid a fixed salary or by hourly rate to determine how earnings are calculated.
Annual Wage*
For salaries, enter the worker's total annual salary amount for the year; this will be divided based on the pay schedule.
Hourly pay rate*
For hourly, enter the worker's regular hourly pay rate used to calculate earnings based on hours worked.
Pay Schedule*
Select how frequently the worker is paid: weekly, bi-weekly, semi-monthly, or monthly.
Pay Period Start*
Enter the first date of the work period covered by this paystub.
Pay Period End*
Enter the last date of the work period covered by this paystub.
Payday*
Enter the date when the worker receives payment for this pay period.
No. of Hours
Enter the total number of hours worked during this pay period, important for hourly workers.
Check Number*
Enter the check or payment reference number for tracking and record-keeping purposes.
Hide Check Number on Paystub
Check this box to exclude the check number from appearing on the generated paystub document.
Earning Type
Select or enter the type of earning such as regular wages, overtime, bonus, commission, tips, or other compensation.
Taxable
Select this if the earning is subject to federal, state, and local income taxes (e.g., regular wages, bonuses, overtime).
Nontaxable
Select this if the earning is exempt from income tax withholding (e.g., certain reimbursements, benefits, or allowances).
Deduction Type
Select or enter the type of deduction such as health insurance, 401(k), garnishment, or other withholdings.
Pre-Tax
Select this if the deduction is taken before taxes are calculated, reducing taxable income (e.g., 401(k), health insurance, HSA).
Post-Tax
Select this if the deduction is taken after taxes are calculated and does not reduce taxable income (e.g., Roth IRA, garnishments, union dues).
Employee Taxes
Click Calculate Taxes to automatically compute all applicable federal and state taxes.
